Role of PET in Ischemic Heart Disease: Myocardial Flow Reserve the Prognosticator

Cardiac positron emission tomography (PET) has the ability to give absolute myocardial blood flow in stress and rest conditions and thereby the ratio known as myocardial flow reserve.

Cardiac PET imaging has changed the scope from advanced and obstructive CAD lesions towards:

  • Subclinical CAD
  • An improved characterization of the extent and severity of

CAD burden in multivessel disease

  • Unravelling of “balanced” reduction in myocardial blood flow
  • Prognostic marker.

Normal myocardial flow reserve obtained with cardiac PET associated with normal perfusion has a warranty period of 3 years from major adverse cardiac events.

Myocardial flow reserve has incremental value over myocardial perfusion for prognosis.
